Alacrity VCC Form
What is the VCC form?
The Violent Crime Control form is a mandatory attestation form required by the Alacrity Managed Repair Network.
The form is required on a yearly cadence and should be filled out by any employees or subcontractors who go to policyholder homes or have access to policyholder data for the Alacrity network.
Sublynk provides a simple way for these forms to be sent to your employees, 1099's, and subcontractors through the Sublynk app. Using the Sublynk system for VCC forms provides:
- A simple digital process for filling the forms out by the personnel who work with you
- Centralized tracking and maintenance of these forms
- Automatic syncing of completed forms to the Alacrity systems
Below are step by step instructions for viewing, managing, and sending the VCC forms to your personnel.
Locating the VCC form in Sublynk
An organization must be actively enrolled in the Alacrity network on Sublynk to send VCC forms.
You can view your Alacrity network status on the dashboard, and you must be in a credentialing or compliant status with Alacrity to send your VCC forms.
Click on the Personnel table to send your VCC forms.
Viewing VCC Form Status & Sending
Once you are in your personnel table you can view the last VCC form status for anyone on your personnel table. You can also send a new VCC form using the action dots on the right side of a personnel table entry:
Once you have clicked Send VCC Form you will see a confirmation screen where you should verify the individual contact information for the VCC form you are sending:
Completing the VCC Form
The VCC form will arrive by email to the contact email provided to Sublynk for the personnel member. That individual will receive an email with a link they need to click on to complete the form:
Once the form is opened the individual can complete the form on mobile or desktop browsers:
After the individual completes the form it will be updated in the Sublynk system. Your personnel table will reflect the latest completed VCC form time and date. Alacrity will also receive a copy of that VCC form automatically for their records.
